Every organization has forms, ours is no exception. Most of the forms
listed below are .pdf documents which require Adobe Acrobat Reader. You
may download a free version of this software (PC, Mac, Linux, Unix

Troop Permission Slip |
We require a signed permission slip for every Troop outing your Scout
participates in. It’s one of those mandatory things. Print out this
form, fill in the details – name and date of the event, Scout’s name,
medical references, emergency information & SIGNATURE, and turn it in to
the individual who is leading the trip. The trip leader must have the
signed form in his/her possession for the duration of the event.

Medical Class 1 & 2 |
Class 1 — updated annually for all Scouts
Used for day and overnight hikes or other programs not exceeding 72
hours. Current personal health and medical summary (history) is attested
by parents to be accurate. The Troop requires this form when a Scout
joins, and annually thereafter in conjunction with Registration. We keep
the form on file for easy reference.
Class 2 — required once every 36 months for Scouts and Adult Leaders
under 40 years old.
Used for activities such as backpacking, tour camping, or recreational
sports involving events lasting longer than 72 consecutive hours. This
form requires a medical examination and physician’s signature.
Note: These forms are mandatory. Missing or out of date forms will
disqualify a participant from an activity. No exception.

Medical Class 3 |
All Class 3 activities require a health examination within the past 12
months by a licensed health-care practitioner. This includes youth and
adult members participating in high-adventure activities, athletic
competition, and world jamborees.
Annually, this form is to be used by adults over 40 for all activities
requiring a physical examination and applies to all Wood Badge
participants/staff regardless of age.

Local Tour Permit |
Prints on legal size paper (8½ x 14)
Used for trips of less than 500 miles. If destination is 500 miles or
more one way or outside the
U.S.A. (local council camp
excepted), use National Tour Permit Application. If backcountry trip, be
sure to know BSA Wilderness Use Policy.

National Tour Permit |
Prints on legal size paper (8½ x 14)
A National Tour Permit is required for all groups traveling to areas 500
miles or more one way from home area (local council camp excepted), or
crossing national boundaries into the territory of other nations.

Merit Badge Counselor |
This form should be turned in to the Troop Merit Badge coordinator along
with a completed adult registration form if you are not already a
